Please Note:

• We understand the importance of anonymity to ensure safety for all tenants. 

• No complaint / grievance will be documented or considered until this form is completed. 

• All complaints must be submitted within 3 weeks of the incident. 

• This form must be completed for our company to consider it, take action, or move toward finding a solution

• Management will issue a notice to the offending tenant that they have a complaint / grievance against them. This notice will not mention the Tenant who reported the complaint or grievance.

• The offending tenant will have 8 business days to respond to the complaint / grievance. 

• If the offending tenant does not respond within 8 business days, any action taken will be at the discretion of Hayes Properties

• If the offending tenant does respond to the complaint denying or disputing the complaint, Hayes Properties will determine a course of action.

• We encourage all tenants to please report all police or fire department visits to their apartment complex to ensure safety for all.
